Answer
- Go to the forms tab on the job in Mitigate (fka MICA)
- Fill out the form and save your changes
- Export the loss to the Mitigate (fka MICA) cloud
- Go to the reports tab in Mitigate (fka MICA)
- Click upload to XA and choose the Work Auth and Forms tab
- Then click Download (to make sure it's filled out) or upload to XA
- Next, verify that it says sent to XA in XA tab
